November 13, 2017 Reminder Open Season started today November 13, 2017 and ends on December 11, 2017 so GET STARTED ! For those of you who have the NALC Health Benefit Plan and either never received your CVS Extra Care Health card or may have lost your card call 888-543-5938 to request a card. Also when you have the NALC Health Benefit Plan and you take the Health Assessment when completed you will be given a choice of: 1 Enrollment in Cigna Plus Savings discount dental program self only when one completes the Health Assessment and family Cigna Plus Savings discount dental program when two or more complete the Health Assessment NOTE you must be 18 or older to complete the Health Assessment any questions call 877-521 0244. 2 Waiver of two $20 medical office visit co payments when one covered member completes the Health assessment Waiver of four $20 medical office visit co payments when two or more (including the member) complete the Health Assessment. 3 A wearable activity tracking device (limit 2 devices per enrollment) To take the Health Assessment go to www.nalchbp.org click on High Option Plan then click on Health Assessment you will need to register with an ID and Password if you have taken the Health Assessment in the past your information should be saved.
